Complex Discharge

The Integrated Discharge Team (IDT) has the skill to help even the most complex patients leave your ward in a timely manner

It is vital you flag up, as early as possible, patients who have complex conditions that will prevent them going home in a straightforward way.

The Integrated Discharge Team will be present at your board round. Its members are regularly on your ward. They can help identify the correct pathway for patients with complex conditions.

If the IDT are involved early, they can help ensure that even patients requiring highly complex care packages, can still leave when they are medically fit.

Not only is this good for patients it also frees up beds on your ward and improves the flow of patients through the hospital.
